It is our mission to provide you with affordable health care, so whether you have insurance, receive financial assistance or are self-pay, we will work with you to ensure you and your family receive a good value for your health care. As a Federally Qualified Community Health Center, we will provide all with healthcare services regardless of ability to pay.

Desert Senita Community Health Center offers a variety of services whether you are uninsured or underinsured. We offer eligibility assistance for programs that may help reduce the costs of these services.

Sliding Fee Discount Program

Desert Senita Community Health Center offers the Sliding Fee Discount Program (SFDP) for services rendered at all of our locations. The Sliding Fee Discount Program is offered to all patients regardless of their ability to pay. Itis solely based on family size and income. The Sliding Fee Discount Schedule is applied uniformly, consistently, and equally for all eligible patients.

Desert Senita Community Health Center offers the Desert Senita Sliding Fee Discount Program for services provided at our facilities. This discount is offered to all patients who may be uninsured or underinsured and is based only on family size and income.

The Sliding Fee Discount Program reduces the cost of services at any of our Federally Qualified Health Center (FQHC) facilities, including our Dental office and Pharmacy.

Our Eligibility Specialist will pre-screen your information and inform you of other programs you may apply for, such as the Health Insurance Marketplace or AHCCCS (Medicaid). You may also speak to our Business Office regarding costs and payment plan options.

In order to apply for the Sliding Fee Discount Program, you will need to provide proof of family size and income. A list of documents will be provided for you so you can prepare for the eligibility appointment. The Eligibility Specialist will go over the list with you and will help you apply in-person.

Once approved, the Sliding Fee Discount Program is honored for one year. After that, you will receive a reminder letter from our Eligibility Specialist reminding you to set an appointment to re-enroll. You will need to provide documentation every time you renew your discount.

The Sliding Fee Discount Program will be based only on your family size and income. We have different discounts ranging from A-D. The Eligibility Specialist will go over the fees once you apply and are approved.

Good Faith Estimate

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance, or who are not using insurance, an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency tems or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least one (1) business day before your medical service or item if the appointment was scheduled three to ten (3-10) business days in advance. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service when an appointment is scheduled three (3) or less days in advance. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
